Planning Your Visit to Pediatric Urology
Overview
If your insurance requires a referral from your child's primary care doctor, you must contact them to request a referral. Please bring the referral form with you to your appointment, unless you have confirmed that our office has received the referral. Your doctor's office may fax the referral to us at (615) 936-1061. We are unable to see patients without a referral.
Your first visit
Please arrive about 30 minutes before your appointment to complete paperwork.
Bring the following to your first visit:
- Your child's Social Security information
- Insurance card
- Doctor's referral form
- Emergency contact information
- Medical records or recent lab work related to your appointment (please bring actual X-rays, not just reports)
We have a small playroom for children to play while waiting; however, suggestions for items you can bring include:
- Crayons and coloring books
- Picture books or sticker books
- Snacks, juice
- Small toys or handheld games
Return visits
Your pediatrician or primary care doctor may have scheduled your first visit. To schedule future appointments, x-rays, or surgical procedures, contact our office at (615) 936-1060.
You should bring the following to every visit:
- Insurance card
- Change of address or telephone number
- Information from your Primary Care Physician's office related to your child's visit
Hospital stay
Sometimes patients must be admitted to the hospital. Most often, these children have undergone surgery. Two adults may stay overnight with a child. Friends and family members should arrive during normal visiting hours from 9 a.m. to 9 p.m. A child life specialist at Children's Hospital can help arrange a pre-operative visit to prepare you and your child for surgery or a stay in the hospital.
You receive discharge instructions and a time and date for your next clinic appointment from your hospital nurse. Call the clinic at (615) 936-1060 If you have any questions about medications, or condition. If you call after business hours, the answering service will contact the on-call doctor.
